Patients booked to have a day surgery procedure must complete a registration and admission form. Completing this form online helps to make the admission process smoother for yourself and the day surgery staff:

  • Submit your form at any time of the day – no need to bring in paperwork to the day surgery
  • Less likelihood of the day surgery staff calling you to follow up missing information
  • Faster processing time
  • All personal information is transmitted securely and confidentially.

Please complete this form AT LEAST 7 days before your admission to avoid unnecessary delays to your procedure.

Tips for completing this form online

  • If you would prefer to use a larger screen to complete this form (instead of a mobile phone) you can use a desktop or laptop computer by visiting visionhospitalgroup.com.au/patient-admission (or ask your day surgery to email you the link).
  • Allow around 20–30 minutes to complete the form.
  • The form must be completed in one go – it CANNOT be saved and returned to at a later time.
  • Have the following information handy before you start:
    • Medicare details
    • Health fund, DVA or workers’ compensation details
    • Advance Care Plan (ACP), Advance Care Directive (AHD) and Enduring Power of Attorney (EPOA) documents (if applicable)
    • List of your current medications, including their dosage and frequency
      • If you take blood thinners, GLP-1 medications (such as Ozempic, Wegovy, Mounjaro, or Victoza) or SGLT2 medications (such as Jardiance or Forxiga), please include requested information and the date of your last dose
    • List of your medical conditions and/or operations and date diagnosed/performed
    • Date of surgery (if you have more than one operation planned, enter the date of the first operation).
